Added banner to the top of "Costco" category that reads "PER COSTCO SENIOR OPERATORS, THESE ITEMS SHOULD NOT BE RESOLD TO COSTCO MEMBERS."

At checkout when the user gets to the shipping method a popup appears saying "Your order will be processed and shipped from our Salt Lake City, Utah warehouse in 2 - 3 business days. Please allow an additional 2 - 5 business days for shipping transit time depending upon where you are located." The user must click "I Agree" in order for the "Continue" button to work in the Shipping Method section. If they click "Cancel" or close the popup, that continue button does not function.

SKU added to product thumbnails underneath product name.
